Author(s) Prof. Pierre-Louis Bras

Abstract Digital health platforms have emerged as important technological infrastructures for supporting patient-centered care management by connecting patients, healthcare professionals, clinical information, communication services, and analytical tools within integrated digital environments. These platforms can incorporate electronic health records, telemedicine, mobile health applications, remote monitoring systems, patient portals, artificial intelligence, cloud computing, and Internet of Things technologies. Their integration can improve information accessibility, communication, care coordination, patient engagement, and personalized healthcare delivery.
This study examines digital health platforms for patient-centered care management, focusing on platform architecture, digital communication, health information management, remote monitoring, personalized services, and intelligent decision support. A qualitative descriptive and conceptual research design was adopted using secondary academic literature. The analysis indicates that digital health platforms can facilitate continuous communication between patients and healthcare providers, support remote care, improve access to health information, and strengthen coordinated management of healthcare services.
However, challenges related to interoperability, privacy, cybersecurity, digital literacy, data quality, accessibility, technological costs, and organizational readiness may affect successful implementation. The study emphasizes the importance of user-centered design, secure data management, interoperable systems, appropriate clinical integration, and continuous evaluation. Overall, digital health platforms can contribute to more coordinated, accessible, personalized, and patient-centered healthcare when technological innovation is aligned with clinical requirements and patient needs.
